Frequently Asked Questions

Upon arrival, you will check in with the front desk.

If you have not called in advance and filled out the online forms in advance, you will be asked to fill out the online forms and schedule an appointment to come back.

If you have pre-filled out your paperwork, you will be aked to wait in the lobby.

With you paperwork pre-filled out online, your wait should be an average of 5 minutes.

Of course we can! When getting treatment from us, there are payment plans that are available to you.

If you are involved in a personal injury case, you can recieve treatment on a lien.

Every recovery is dependant on the injuries sustained, and the way they are treated. Because of this it is important to have a team of specialists who have good communication, and understand what it is that your injuries need, as well as what documentation your legal team will need if you are in a suit.

You can rest easy as all the specialists you need are all working in close contact and proximity of each other, which means solid communication, less commuting around for treatments, and a more streamlined treatment process for you!

The answer to this question is: Absolutely!

There are a few nuances of working on a lien, and not every place knows them, or is happy to go through the hassle of setting them up

Because your medical needs are our utmost priority, you will find us going the extra mile to help navigate these nuances on your behalf so you can focus on getting better faster.

When it comes to the topic of paying for your treatment, there are four ways things get paid for:

Your Personal Health Insurance: Your personal health insurance coverage is the primary source of coverage for any medical bills arising from the treatment of car accident injuries. It’s going to work the same way that it does for any other covered medical expense that you incur in your day-to-day life.

Don’t be worried about using your insurance, this the reason why you decided to purchase health insurance coverage to begin with. However, you should be aware that if you receive a settlement or judgment, your insurer will be entitled to recoup all payments it made on your accident claims.

The Other Driver's Auto Insurance: In the case of you not having health insurance coverage, you may be able to have your medical treatment costs covered by the auto insurance policy of the at-fault driver.

Keep in mind auto insurance policies only cover medical expenses up to the amount stated on the policy, so if the other driver's policy has coverage up to $100,000, and your treatment costs add up to $150,000, you will be personally responsible for the $50,000 that exceeds the other driver’s coverage.

Uninsured/Underinsured Motorist Coverage: Arizona law requires all drivers to carry auto insurance with a minimum bodily injury liability coverage of $15,000 per person and $30,000 per accident. Sadly, a high percentage of Arizona drivers don't follow this law.

If the driver who caused your accident had no insurance or had insufficient insurance to pay for your injuries, you can look to your own uninsured/underinsured coverage to pay your medical bills.

No Insurance Coverage of Any Kind: If you don't have any insurance coverage, your medical bills will have a lien placed on them. This lien will be paid off upon reaching a settlement for your case.

In the state of Arizona, there are some nuances when it comes to Medicaid (AHCCCS) and Medical Liens in Arizona. Some hospitals try and place liens on treatments for their personal injury patients that AHCCCS had already paid out for, this is in an attempt to recover the balance between what the AHCCCS paid and the rate the hospital usually charges.

This practice is called balance billing, and when you’re dealing with Medicaid (AHCCCS), it’s an unlawful practice. The medical provider can only accept the lien or Medicaid (AHCCCS), not both.

Testimonials

HAPPY PATIENTS:

Who Are We?

ABOUT AZURE HEALTH:

Azure Health, your one stop medical shop for all things Orthopedics, Total Joint Replacement, Sports Medicine, Pain Management, & Sports Performance Training.

You may be asking:
"How do you do all this for me, without sending me to several places?"
That's a great question! This is possible due to Azure Health being a multi specialty group.

Curious as to what that means?
A multi specialty group is: A clinic where doctors work together like one big family. They achieve this by sharing the same facilities, administration, income, expenses, support staff, & equipment.

Wondering how this will benefit you?
This allows you to come to one place for each phase of your treatment, however big or small. Because you are seeing specialists and physicians, who all work together in the same facility day in and day out. Your treatment providers will have better communication, be used to effectively working together, and in turn provide you a higher level of treatment.

So what are you waiting for?
